  • I knowing and willingly consent to have services preformed at Clean Beauty Boutique during the Covid-19 Pandemic. I understand the COVID-19 virus has a long incubation period during which carriers of the virus may not show symptoms and still be highly contagious. It is impossible to determine who has it and who does not, given the current limits in virus testing. I understand that due to the frequency of visits of other clients, the characteristics of the virus, and the characteristics of salon/spa services, that I may have an elevated risk of contracting the virus simply by being in the salon. I confirm that I am not presenting any of the following symptoms of COVID-19 listed below:

    Fever

    Cough

    Shortness of breath

    Chills

    Repeated shaking with chills

    Muscle pain

    Headache

    Sore throat

    New loss of taste or smell

    To prevent the spread of contagious viruses and to help protect each other, I understand that I will have to follow the salon’s strict guidelines.

    I understand that the CDC, OSHA, and state of Illinois recommend social distancing of 6 feet.

    I verify that I have not traveled outside of the United States in the past 14 days to countries that have been affected by COVID-19.

  • We respectfully require 36 hour notice for cancellations or changes.

    Late Cancellation: Any cancellation made less than 36 hours before the scheduled appointment will require a credit card on file and prepayment for rescheduling.

    No-Show Appointments: For appointments that require no shows, a credit card on file is required along with pre-payment of the appointment before booking.

    Excessive Rescheduling: If there are more than 3 consecutive appointments that need to be rescheduled, a credit card on file and pre-payment of the appointment will be required before booking.

    If you arrive more than 10 minutes late to your appointment, you may have to reschedule your service(s).

    If you no-show or cancel your appointment we cannot guarantee that you will be able to rebook immediately with your preferred stylist.

    Credit cards will be put on file for color corrections or vivid colors.

  • ​The safety of your little ones is our utmost concern.

    In order to create a safe and relaxing atmosphere, Clean Beauty Boutique requests only well-behaved children over the age of 13 to accompany adults to appointments, if necessary. Those under the age of 13 who are not receiving a service are kindly asked to refrain from visiting Clean Beauty Boutique.

    Our salon is not child-proof. Sharp instruments, hot tools, and chemicals make for a potentially dangerous environment for young children.

    We apologize for any inconvenience.

  • Clean Beauty Boutique is not responsible for any lost or stolen items and any damage to clothing during color services.

  • ALL EXTENSION SERVICES ARE FINAL SALE. Extension services cancelled within 2 days of the scheduled appointment may result in a non-refundable cancellation fee* depending on the type of service booked.

    If you chose to go over the recommended 6-8 week maintenance time frame for hand tied or tape extensions, you are subject to Clean Beauty's premium pricing which is at 1.5% of the total service cost. In an extreme case if maintenance is done properly we will do a removal but we may not reapply the hair extensions.

    If you chose to go over the recommended 4-6 months time frame for fusion extensions you will be subject to Clean Beauty's premium pricing at 1.5% of the total removal cost. Waiting longer then recommended to remove fusion extensions can cause extreme matting and possible scalp damage.

  • ​Clean Beauty Boutique is not 100% organic. We strive to use the most organic-based products on the market, but unfortunately not every product can be made or manufactured as 100% organic. We provide clients with the most clean, safe, and organic-based options currently available.

  • ALL SERVICES ARE FINAL SALE unless there's a mistake from the service provider... not something you might have changed your mind on (after the service has been performed). In case of a disagreement, a redo must be validated by management. You have 2 days to contact the salon to receive a complimentary service to adjust any dissatisfaction. Corrective services will not be allowed 5 days past the initial service date. All corrective services will be preformed by the original stylist to be complimentary.

  • Hair care can be returned up to 2 weeks after purchase date for a refund.

    All cosmetics and skincare are FINAL SALE.
